To book an appointment

please start by filling out this initial contact form that helps me understand your needs a little further and determine if we are a good fit.  After reviewing this information, I will reach out to you to discuss whether my practice is well equipped to help you achieve your goals, and if so, help you schedule your first appointment. 

Phone Contact

For established patients, I would like to know about any problems or discomfort you are experiencing with medications. Please leave me a message as I generally return calls within 24 hours, except on the weekends.  If your urgency is so acute that waiting for a response from me is not appropriate, or, if you are concerned about your safety or the safety of someone else, please call 911 or go to the nearest Emergency Department.  Brief conversations on the phone to discuss symptoms and side effects will be held at no charge to you.  If the discussion is complex, an appointment will need to be scheduled.  

Refills

You will receive new prescriptions and appropriate refills during your appointment time. In most cases, additional refills will not be provided to pharmacies by phone without an appointment or by prior agreement. Please contact me to make an appointment when you pick up your last available refill.
